_Registration 

On your first visit, please be ready to provide:

• Photo ID
• Proof of Income
• Proof or Address (gas or electric bill or rent receipt)

For every visit, please bring:

• Immunization Record
• Medications being used or recently used
• Medi-Cal card health coverage information

If you are unable to keep your scheduled appointment, please notify us at (562) 933-0400 as soon as possible so that we may open up the slot for another patient.
