FOR TREATMENT OF MINORS |
- A parent or guardian must sign a consent
for the treatment of a minor (under 18 years of age)

Clinic visits are by appointment
only. If you or your child are sick and need to be seen on the same day,
please first call the clinic nearest your home. Hours
In case of emergency, please call 911.

| CANCELLATIONS |
| If you are unable to keep your scheduled
appointment, please notify the Clinic where your appointment is scheduled as soon as possible. |
| FEE
FOR SERVICE |
| For those who qualify due to limited
income status, The Children’s Clinic offers free healthcare
services. For others the fee schedule is on a sliding scale, based on
the ability to pay.
Rates are established by Financial Counselors at the time of your visit.
There may be additional charges for laboratory, X-ray, other diagnostic
tests and medications. Patients with a current fee for service Medi-Cal
cards are charged only for non-covered services and medications only. |

MANAGED CARE |
| Patients who currently have or have signed
up for Medi-Cal Managed Care and Healthy Families may select The
Children’s Clinic as their Primary Care Provider.
