A dedicated team of Masters in Public Health individuals, Chronic Disease Educators, Community Health Workers and AmeriCorps interns, TCC’s Health Education and Outreach (HEO) is always on the go to make a difference in our patients’ lives! HEO strives to partner with our community to provide the most culturally and linguistically appropriate health education and promotion on the topics that are most relevant to our patients and our community.
They also work very closely with TCC patients and families to help them make healthy lifestyle changes when they are ready to make them. Team members are located at all nine of our clinic sites, while also working in the community and in some cases, visiting our patients in their homes. The team provides a multitude of programs that complement and enrich the high quality health care services TCC provides. By helping and encouraging patients to be active and informed when interacting with their provider, TCC empowers patients to take charge of their health take charge of their medical care and health. The HEO team also offers no cost health promotion and outreach opportunities to the community at large.
